Welcome! You are why we are here! SAM Health is able to care for most of your primary care needs under one membership cost. We have upfront pricing so you’re never stuck with a surprise bill. 

The direct primary care model offers you access to a physician who knows your story. So even if you don’t go to the doctor often, having piece of mind that you can contact us instead of spending hundreds of dollars and minutes at an urgent care or waiting weeks for an appointment is why we are here! We will also remind you when it’s time to check for chronic diseases and we will keep up with your cancer screenings. Feel free to call us to discuss how a direct primary care membership can serve you! We’d love to chat. 

YES! We offer one time visits, but it doesn’t get you all the awesome added benefits of being a member. Consider joining! 

YES! When determined by you and your doctor that a telemedicine appointment would be best for your needs, we can send you a link to join. And don’t worry, this is also included in your membership. 

Primary care can cover about 85-90% of your needs. For anything else, like imaging and specialists we will work with our local partners to get you the highest quality care and most affordable rates. 

Just let us know 30 days in advance and we can cancel your membership. Please let us know if there’s something we can do to keep you in our family!
